Information for GPs

Referring patients: Mental health / The Southwest Clinic

Referrals are received from psychiatrists, general practitioners, health professionals, carers and self-referrals.

Referrals to the Unit can be directed to Nursing Unit Manager or the nurse in charge for after hours admissions on (02) 9600 4340 / 9600 4278.

An intake form can be filled out with the required information and directed to health insurance companies for approval.
